Certification Statements for SUNY Campuses

Certification Statement: Compliance with Title IV.
This statement should be attached to the Executive Summary of the Self Study.

Community Colleges 

State-Operated Campuses

Certification Statement: Compliance with MSCHE Requirements of Affiliation and Related Entities Policy.
This statement must be signed by the Chair of the SUNY Board of Trustees.

At least sixty days before the form is needed, campuses should send a signed copy to:

The Provost’s Office will obtain the Chair's signature and arrange for the timely return of the document to the campus.
