Thank you for filling out Senator Gillibrand's Fiscal Year 2010 Non-Defense Appropriations Request form. This application is due by close of business on Friday, February 20, 2009. No exceptions will be made. Applications received after this date may not be eligible for submission. In, addition incomplete forms will not be accepted. If you do not know what to write in a blank, take your best guess and we will contact you if we need more information. Early submission is encouraged. Due to increased federal deficits, increased competition, and increased scrutiny over earmarks, securing an earmark in the annual federal appropriations bills has become more and more difficult. While we are pleased to receive your request for funding, we cannot guarantee your project will be funded. We highly recommend that your organization consider applying for federal grants as well. INSTRUCTIONS FOR FILLING OUT THE FORM Section I (Organization Information) Please include the following information: * Name of Submitting Institution * Address * County and City * Phone number * Website Section II (Project) Project Title Project Description: Do NOT give a summary of what the organization does but rather a VERY SPECIFIC one to two sentence description of exactly what the money will be spent for. (Capital improvements, salaries, teacher's supplies, etc.) Amount of funding requested: Appropriations bills are legislation that are passed annually. Multiyear funding is not possible in an appropriation bill and you should list how much funding you are requesting just in Fiscal Year 2010. Project's total cost: how much does your ENTIRE project cost, not how much are you requesting from the Federal Government. Priority: If your institution or organization is submitting more than one funding request, we ask that you prioritize them. #1 being your highest priority. Why does your project warrant federal funding?: Write 2-3 sentences to support why federal funding for this project would benefit the community and help to create jobs during this critical economic time. President's Budget: The first Monday in February of each year, the President puts out his budget for the following fiscal year. If you are requesting funding for a very specific New York organization and a very specific earmark, there is a better than 99% chance that your project is NOT in the President's budget. www.OMB.gov is where this budget document can be found. Is your organization for-profit or is it a not-for-profit? If seeking funding for a transportation project, is your project on the Statewide Transportation Improvement Program?: The STIP is a listing of projects eligible for federal funding. Information relating to the STIP can be found at https://www.nysdot.gov/programs/stip If seeking funding for a water or sewer project, is the system currently under state or federal consent decree?: What is the estimated start date for your project?: Please list the counties that will benefit from your project: Please list the current number of employees at the organization: Please list the approximate number of jobs this funding would create: Section III (Sources of Funding) Please identify the specific appropriations bill and the specific account that your project would qualify for. This will require some homework on your part where you will need to research how your project meets the qualifications of the specific account of the Appropriations bill. Which Appropriations bill does your request fit into? There are 12 Appropriations bills: 1. Agriculture, Rural Development and Related Agencies (AG) 2. Commerce, Justice, Science and Related Agencies (CJS) 3. Defense 4. Energy and Water Development* (E&W) 5. Financial Services and General Government 6. State, Foreign Operations, and Related Agencies (Foreign Ops) 7. Homeland Security** 8. Interior, Environment, and Related Agencies* 9. Labor, Health and Human Services, and Education (Labor-HHS) 10. Legislative Branch (Leg Branch) 11. Military Construction, Veterans Affairs, and Related Agencies (Mil-Con) 12. Transportation, Housing and Urban Development* (TTHUD) PLEASE NOTE: * = may require local matching funds **= The Homeland Security Appropriations bill traditionally does not provide congressionally directed projects. As we begin the FY09 cycle, we are uncertain if this practice will continue or if they will allocate funding for Member projects. Please take this into consideration when choosing the most appropriate bill within which to make your request. Under which account do you seek funding?: For example: Rural Utilities account in the Ag bill; or Economic Development Initiatives account in the THUD bill. House Members making your request: Please be as specific as possible. If you have not yet contacted your House Member, please do so. A request with House and Senate support is more likely to receive funding as a request with support from only the House or the Senate. Please include the House Staff member that you are working with. Other Sources of Funding: You increase your organization's chances of funding if you can show that your project has local, state and private monetary support. Any State or local government dollars, coupled with any private fundraising dollars will show that your organization has local presence. Please list all anticipated sources of funding followed by any funding that you have already received. ** As you come across additional funding, please contact our offices to update your request** Section IV (Past Funding) If you have previously received federal earmarked funding for this specific project that you are requesting funding for, please list the amount of funding this project has received over the last four fiscal years. Please list other sources of federal funding this project has received (Federal competitive grants, contracts, etc). Please also list any other earmarks your organization overall has received from the federal government. If required, please list what matching funds will be made available. Please list other projects your organization has requested during this funding cycle. Section V (Contact Information) Please list all relevant contact information for this project. Section VI (One Pager) Please do not forget to ADD a one-page summary directly to the application (not as a separate attachment) that describes your proposed project and explains how the project meets the criteria of the bill and program title that you have selected and how the project benefits the region in the State of New York. Section VII (Budget) Please provide a detailed budget of this project. Section VIII (Letter of Support) Please attach ONE letter of support from an agency/local government official/organization that supports your request for funding. Only the first letter received will be accepted, any additional letters will be discarded. For defense projects, a letter from the military is preferable, but a letter from a local government office will also suffice. Please know that while the appropriations process begins early in the year, the vast majority of bills will not be considered until the end of the calendar year.
