Registration Fees for Seminars or Conferences

The department completes the registration form and attaches it to the purchase requisition. Also attach any seminar information, such as brochures or agendas.

In the description section of the requisition, state the name and title of the person who will be attending and the date, time and location of the seminar/conference. If the registration was already made via the phone or fax, please indicate on the requisition that it is "confirming". This will avoid duplicate payment.

All registrations that are to be prepaid by check must include the name of an alternate attendee, in case the original attendee is unable to attend the conference or seminar.
